YOUR ASSUMPTIONS

Make the intended change clear.

Set a baseline and a target, then write down who should benefit and how you will check the result.

View your live scorecard

Starting value for one defined measure. Use the same units, population and measurement method as your target.

400

A goal for that measure, rather than a claim of achieved or caused impact.

250

Choose whether a higher or lower value represents the intended improvement.

Decrease

A working name is enough.

Not entered

Be specific about who is included and who may be left out.

Not entered

Use the same unit and definition for the baseline and target.

Not entered

Comparable periods make a change easier to interpret.

Not entered

Separate measured results from forecasts and assumptions.

Not entered

Consider unintended effects and who might bear the risk.

Numbers describe an intended target, not an achieved result. Text and numeric entries stay in this page; print your scorecard before leaving.

The numerical target aligns with your chosen direction: decrease. Alignment is not evidence of benefit or causation.

This scorecard records your assumptions and target. It does not establish an achieved outcome or show what would have happened without your work. Review evidence, alternative explanations, and unintended harm before making an impact claim.

UNDERSTAND THE MODEL

Useful numbers.
Clear assumptions.

Signed planned change = target − baseline · Relative planned change = (target − baseline) ÷ baseline × 100, when baseline is not zero

Choose whether an increase or a decrease is desirable for your measure. The direction check tests the arithmetic against that choice; it does not determine whether the measure represents a meaningful benefit. Keep the measurement definition, unit, population, and time periods comparable.

A before-and-after difference does not establish that your work caused the change. Outcomes may reflect other factors or changes that would have happened anyway. A credible impact claim requires evidence about who experienced what, how much, your contribution, and the risk that results differ from expectations. This worksheet is a planning aid, not a verified impact assessment.
